What is an Evidence of Coverage?
The “Evidence of Coverage” (EOC) is a detailed document that explains how to get your Medicare drug coverage through our Blue MedicareRx (PDP). This document, together with your enrollment form, riders, formulary and the amendments that we may send to you, is our contract with you. The EOC also explains your rights, benefits and responsibilities.
The EOC document explains:
- What is covered by Blue MedicareRx (PDP) and what isn’t covered
- How to get your prescriptions filled, including some rules you must follow
- What you will have to pay for your prescriptions
